TutkimusSuomessa on tällä hetkellä arvioiden mukaan n. 4 000 5 000 opioidien väärinkäyttäjää, joista n. 2.200 on ns. lääkkeellisen korvaushoidon piirissä. Korvaushoito on lukuisissa tutkimuksissa todettu tehokkaimmaksi opioidiriippuvuuden hoitomuodoksi.
Siitä huolimatta suhtautuminen tähän hoitoon on edelleenkin ristiriitaista, mikä on myös näkynyt Suomen huumausainepolitiikassa sekä hoitojärjestelmän kehittämisessä ja kehityksessä. Erityisenä huolena on edelleen korvaushoitolääkkeiden buprenorfiinin ja metadonin väärinkäyttö tai niiden päätyminen katukauppaan. Muista Pohjoismaista poiketen buprenorfiini on Suomessa eniten suonensisäisesti väärinkäytetty opioidi. Tämä asettaa erityisiä haasteita hoidon kehittämiselle.
